How to Become a Patient


In order to qualify as a patient at CMCAC, you must be an Aiken County resident, (b) not covered by Medicare, Medicaid or Veterans benefits or any other medical or prescription drug insurance, (c) you cannot be a patient of another clinic, and (d) household income has to be at or below 200% of the Federal Poverty Guidelines.

The first step is to contact the clinic for a mini-screening and to schedule a screening appointment. An appointment can be scheduled after the screening process is completed and qualification is approved.


Patient screenings are held on Mondays and Wednesdays, from 1 p.m. – 4 p.m. Please call 803-226-0630 to schedule an appointment.


Office visits are scheduled for every 3-4 months or as frequently as necessary to stabilize a patient.

Submit Required Documentation

  • South Carolina Driver’s License or Personal Identification Card from the Motor Vehicle Department of South Carolina
  • Social Security Card
  • Past year’s income tax papers
  • Current pay stubs
  • Social Security Disability Benefit Statement
  • Food Stamp Benefit Statement
  • Unemployment Benefit Statement
  • Medicaid Denial Letter
