Become a Patient

Join The Community Medical Clinic of Aiken County to receive tailored healthcare designed to meet your individual needs. Starting with a simple mini-screening, we guide you through a straightforward qualification process, ensuring you receive the appropriate care.

In order to qualify as a patient at CMCAC, (a) you must be an Aiken County resident, (b) not covered by Medicare, Medicaid or Veterans benefits or any other medical or prescription drug insurance, (c) you cannot be a patient of another clinic,  (d) household income has to be at or below 300% of the Federal Poverty Guidelines,  (e) have a qualifying diagnosed chronic health condition: high blood pressure, high cholesterol, thyroid conditions, diabetes, seizures, asthma, COPD, or obesity and (f) be between the ages of 19 and 64. 

Anyone 18 years or older may participate in our R.I.S.E. (Real Independence Self-Empowerment) program. Make sure to let the staff know that you are wanting to schedule a R.I.S.E. assessment appointment.

The first step is to contact the clinic for a mini-screening and to schedule a screening appointment. An appointment can be scheduled after the screening process is completed and qualification is approved. Patient screenings are held on Mondays and Wednesdays, from 1 p.m. – 3 p.m. Please call 803-226-0630 to schedule an appointment. Office visits are scheduled for every 3-4 months or as frequently as necessary to stabilize a patient.

Office hours are Monday – Wednesday, 9:00 a.m. – 12:00 p.m. & 1:00 p.m. – 5:00 p.m., and Thursday, 9 a.m. – 12:00 p.m. & 1:00 p.m. – 3:00 p.m.

Submit Required Documentation:

  • South Carolina Driver’s License or Personal Identification Card from the Department of Motor Vehicle of South Carolina
  • Social Security Card
  • Proof of Household Income: Current pay stubs, past year’s income tax papers, everyone in households combined income.
  • Proof of Address: Any document confirming your current address.
  • Social Security Disability Benefit Statement if Applicable
  • Food Stamp Benefit Statement if Applicable
  • Unemployment Benefit Statement if Applicable
  • Medicaid Denial Letter if Applicable

New Patient Paperwork will be provided for you to fill out and return after presenting required documents.
